  1. How do I schedule an appointment?


  1. You may call to speak with Pat who will walk you through a phone intake form. 

  1. What can I expect at the first and subsequent sessions?


  1. You will be asked to plan for a little extra time for the first session to complete forms that will guide our first session.  If you want Pat to submit billing to insurance for reimbursement, please bring your insurance card for photocopying.  If you have a co-payment - please come prepared to pay the co-pay.  If you have a co-insurance plan, I will submit the charges of the session and follow up with you on any balance due.

  1. Working with individuals, couples, and families on life-cycle transitions, relationship changes, and recovery from addiction.


  1. What happens if we don’t connect?


  1. I will be happy to provide referrals for you if desired. 


  1. How do I pay for services?

     a. You may cover fees with cash, check, or by having Pat submit the session charges to insurance for reimbursement. 
        Currently, Pat is a provider for:
    •  BC/BS of MN and some other states in the AWARE and SELECT programs.
    • T.E.A.M. Inc., - EAP
    • Some other private insurance plans


    1. What should I bring to the first session?
